Pathways to Care became an NDIS registered provider of Support Coordination and Plan Management services in 2017, and first opened an office in Blackburn to provide support to participants of the NDIS in the Inner and Outer Eastern NDIS regions of Melbourne.
From humble beginnings and a shared desire to provide a high-quality, independent service to people with a disability, Pathways to Care still continues this ethos. Pathways to Care is now solely focused on providing a high-quality, participant-focused, NDIS Plan Management service, growing to now support participants across Australia.
Providing quality support to participants no matter where they live, whether in a small regional town or a large capital city, is a driving force for the team.
Jane, Sam and Ros’ combined experience, background and qualifications cover Human Resources, Occupational Therapy and Corporate Governance, with all three being passionate about making a real difference to people with a disability. They all still work very closely in the business ensuring all participants receive a high quality level of service, as well as ensuring all team members of Pathways to Care share the same vision and passion for the people they support.
